Please make sure to input the correct information on the timesheet. If the timesheet has any incorrect information, we will need a corrected timesheet in the right format before you will get paid.

Read each box closely to ensure you are inputting the correct information in the correct box.

If you are Option 2 or 3 in Vesta:
– You MUST fill the EVV Confirmation/Visit Maintenance Request form.
– You will either confirm that your time in Vesta is correct OR you will request changes to time clocked in Vesta.

ALL CLIENTS:
– Fill the Non-EVV CDS Timesheet ONLY for services that are not recorded in the Vesta EVV system.

Please use this form to certify your Vesta timesheet OR request maintenance.
Note: You are required to have 80% accuracy on EVV visits. No more than 20% of visits may be entered manually in total across all attendants employed.
