We Provide Personalized Care


Home Health Services

At the direction of your doctor, home health services are provided for short-term medical care to those living in Dallas and Tarrant counties. Services may include skilled nursing care; physical therapy, occupational therapy, and speech therapy; medical social services; home health aides, and more.

Long-Term Care Support

Through our variety of programs, including Star Plus/Star Kids program, we provide long-term care for those with long-term disabilities. Services available vary for each person. It is available for both adults and children whose health problems cause them to be functionally limited in performing activities of daily living.

Intellectual Developmental Disabilities Services

We provide community support services to individuals with intellectual developmental disabilities or autism living in their own homes through contracts with the local Mental Health Mental Retardation (MHMR) centers in Fort Worth and Dallas. The local MHMR center determines screening, eligibility, and referrals.

Attendant Care for Working Individuals

We help bring support to people with disabilities that live and work independently in their communities through the provision of personal attendant care. Referrals are received through the Texas Health and Human Services Commission.

Veterans Home Care

We provide home care for veterans who have served at least 90 days of active duty, and at least one day during a war period. Veterans can receive up to 73 hours of support per month, and surviving spouses can receive up to 47 hours per month.

Private Pay Family Support

We provide in-home care for those who do not qualify for governmental assistance through our, “Your Home….Your Way” program. Families are provided support with activities of daily living and with services customized to their needs.

Care Navigation Center

If you or a loved one is diagnosed with an illness or disabling condition, we’ll connect you with an expert in the resources available (public and private) and help create a plan of action to empower independent living.
